- Describe your family (include names)
- Place(s) of employment for yourself and spouse
- Favorite sport/hobby (please name one for each)
- Why you are interested in becoming a sponsor
- Signatures of both Primary and Spouse
- Must be at least 28 years old to register
Two Letters of Recommendation (from friends/neighbors/co-workers, not family members)
- How long have they known you?
- What is the basis of their relationship with your family?
- Why they feel your family would be a good candidate?
- Please ensure that these letters include signatures.
Grads, Parents of Cadets, DoD, Military
