Parents' Club Leadership Information Release

By completing and submitting this form, you agree to give the U.S. Air Force Academy Parent Liaison Team permission to share your contact information with the Academy Staff, Academy parents, other Academy Parents’ Clubs leaders, and the Academy Association of Graduates (AOG) so they may contact you as necessary. This information will be stored by the Academy Parents’ Liaison Team and shared with the Academy Staff, Academy parents, other USAF Parents’ Club leaders and the Academy AOG during the duration of your club leadership term.

We request all Academy Parents’ Club Presidents, Co-Presidents and Vice Presidents submit a form at the beginning of their leadership term.

Information Release Statement
I consent to allow my name, address, phone number, and e-mail address to be released to Academy Staff, Academy parents, other Academy Parents’ Club Leaders, and the Academy AOG during the duration of my leadership term.
Use of Class Year Cadet/Parent Information Release Rosters
The Academy Parent Liaison Team maintains information release rosters for current Academy classes. Only those cadets and families who have signed release forms and submitted them to the Academy Parent Liaison Team will be included on these rosters. The Parent Liaison Team will only share these rosters with parents’ club leaders who have completed an Academy Parents’ Club Leadership Information Release AND agree to the statement below.

I agree to protect the class year cadet/parent information release rosters provided by the Academy Parent Liaison Team. These rosters may not be shared outside of my club leadership team. I will not share these rosters with other Academy parents or Parent Facebook Page Administrators. The parent liaison team will share rosters with class spirit committees as needed.

NOTE: If you’d like to create a membership roster for your individual parents’ club, you must have members sign a release form agreeing to share their personal contact information with other club members.
