Note: With the exception of walk in services, please call (719) 524-CARE (2273) to access the following:
  • Scheduled appointments with Patient Centered Medical Home Team (Primary Care Managers)
  • Walk in services available Monday through Friday 0800-1100 & 1300 - 1500
      –Blood pressure checks (provider ordered), suture removal, throat cultures
  • Patient education classes - No referral is needed
      –Diabetes education, high cholesterol/high blood pressure education, or asthma education.
  • Behavioral health services (located in Family Practice)
      –The behavioral health consultant works with you and your PCM to address behaviors, thoughts, emotions, stress, and relationships that may impact your physical health and quality of life.

      How do I schedule an appointment?

If you are assigned a Medical Home PCM, you simply need to call 524-CARE and follow the voice prompts to make an appointment with your PCM.

      Medication Refills/Renewals

Request medication refills using the automated pharmacy system by calling 524–4081. Accomplish medication renewals or changes by calling 524–CARE and scheduling an appointment with your PCM.

      How do I obtain copies of my medical information?

A signed medical release of information will be sufficient. These forms can be obtained from the Admissions and Dispositions section.

      How do I obtain a Pregnancy Test/OB consult?

Request a test at the GYN Clinic, fill out the information sheet, and proceed to the lab. A nurse will call you in the afternoon if it is positive. At that time a consult will be placed for OB care with the TriCare Contractor.

      What should I do if I am unable to keep a scheduled appointment?

In order to maintain available appointments for all of our patients, we request that you call 524-CARE to cancel your appointment. By you canceling your appointment, we will be able to provide timely services to our other beneficiaries.

      Clinic Snow Plan

The weather in Colorado can be very unpredictable. During inclement weather, the base will inform the public of any delays or closures through the local news and radio stations. Academy weather information can also be obtained through the base website: Please allow ample travel time and reference the local weather stations and base information prior to reporting for your scheduled appointment. For everyone´s safety, patient appointments will be cancelled during late reporting (delays) and base closures. You may call the appointment care line at your earliest convenience to rebook your appointment.
